Required Sports Physical Information
  • Incoming freshmen and transfer students (that have not previously completed a collegiate sports physical) need to print this form.
  • Athlete should complete and sign section 1 (pages 1-3)
  • Bring pages 4-5 to your physical exam with your medical provider
  • Must be completed by an MD, DO, NP, or PA - sports physicals from a chiropractor are not accepted
  • Incoming athletes will need a sickle cell test result, so if you have never had a sickle cell test, you might want to inquire with your medical provider if that can be included during the sports physical.
  • Make a copy to retain for your records
  • Hold onto the form and await communications later in the summer for how to upload to Moodle, preferably as a PDF. High quality JPG's will be accepted.
  • Our Sports Medicine partner - TCO - offers FREE Sports Physicals for both high school and college student-athletes in Minnesota and Wisconsin. HERE is the information for 2026-27. Registration will open at some point in June, but their tentative schedule is posted. This could be helpful if you received a physical in the last year, and insurance will not cover a second physical.
